LIA Social Determinants of Health Microcredential Program

Intermountain Healthcare and Latinos In Action have partnered up to provide students with the opportunity to get trained as Health Screeners. This micro-credential provides a means to develop skills and a knowledge base for addressing social determinants of health, pre-diabetes, high blood pressure, and depression. The course will help you to gain content expertise and necessary skills to successfully perform health screenings, interpret the collected data, and become a health advocate.

NOTE: This course is not the equivalent of a Certified Nurse Assistant (CNA/PCT) or Medical Assistant (MA) program. This course is not designed for pediatric health screening.

1. Attend training and demonstrate knowledge and skills through class participation

The learner attends and participates from a one hour in person or virtual (live) training provided by Intermountain Healthcare and takes notes on the Attendance and Participation Form to upload as Evidence #1.
An Intermountain Healthcare representative will provide the necessary forms and official Intermountain Healthcare envelope for your students to place them in after completion.

2. Perform and submit a minimum of two health screenings

The learner performs a minimum of two health screenings and completes the forms provided by Intermountain Healthcare. Students will then place the screening forms inside the official Intermountain Healthcare envelope, seal it, and return to the teacher.
Students will then complete the Implementation (Health Screening Verification) Form with teacher’s signature and upload it to the MIDAS site as Evidence #2.

3. Interpret the collected data and complete the Outcome Reveal Report

The learner notifies each adult involved (parents, legal guardians, adult family members) of the outcome of the health screening and writes a one paragraph testimonial report about the results for each adult that participated.

NOTE: This portion focuses on the screened person’s reaction. It could include answers to the following: How did that conversation go? How did they react- with concern, surprise, relief? Did the result cause the adult to want to take any further action such as visiting a doctor, changing eating or exercise habits?

This Outcome Reveal Report must be submitted as one evidence document and uploaded to the MIDAS site as Evidence #3.

NOTE: Even if you screen 20 adults, you will submit the testimonials on ONE document.

4. Complete a Reflection and submit Badge for Review

Students will complete a Reflection essay or video on “How this course has impacted my life.” This will be Evidence #4.

The following questions may be used as a guide to complete the written response or video reflection:

What did you like about this course and the whole experience?
What did you learn through this course?
Did this give you a new perspective, challenge your point of view, or introduce you to new techniques, skills, processes?
What positive contribution did this course make toward your future college or career goals?

Once Evidences #1, 2, 3, and 4 have been uploaded, click on “Submit Badge for Review” to turn in all of the Evidences.

Suggested Microcredential Completion Timeline

HOW TO LOG IN

1. Go to latinosinaction.midaseducation.com/login Your username is the same email address you used to register yourself for this microcredential.

2. Re-enter your email address and click Reset Password.

3. Check your email for a message from MIDAS Education asking you to reset your password. If you don’t see it, check your Spam folder.

4. Create your password with at least 8 characters.

HOW TO DOWNLOAD FORMS

1. Go to latinosinaction.org/social-determinants-of-health . Select any form needed and click “Download.”

2. Scroll down to download other forms you might need.

HOW TO UPLOAD EVIDENCE

1. After logging in, click on “My Profile” on the left hand side.

2. Select “Microcredentials” on the left.

3. Click on the Mirocredential catalog.

4. Click on the “More Info” button.

5. Click on the “Earn This Micocredential” button.

6. Click on the “Start Working” button.

7. Click on the “Latinos In Action Social Determinants of Health” Microcredential.

8. To start submitting evidence click on the “Submit Evidence and Reflection” button.

9. Click on the “Submit Evidence” button.

10. To submit evidence #1 click on the “Attendance and Participation” button.

11. Click on “Choose File” to select evidence file from your device.

12. Type “Attendance and Participation” in the description box. Then, click the “Submit” button on the bottom left corner of your screen to finish your evidence submission process.

13. To submit evidence #2 click on the “Implementation” button.

14. Click on “Choose File” to select evidence file from your device.

15. Type “Verification Form” in the description box. Then, click the “Submit” button on the bottom left corner of your screen to finish your evidence submission process.

16. To submit evidence #3 click on the “Outcome Reveal Report” button.

17. Type “Outcome Reveal Report” in the description box. Then, copy and paste your report in the narrative box, and then click the “Submit” button on the bottom left corner of your screen to finish your evidence submission process.

HOW TO SUBMIT REFLECTION

1. Click on the “Submit Reflection” button.

2. Copy and paste your reflection in the box or click on “Choose a File” to upload a video reflection from your device. Then, click on the “Submit” button on the bottom left corner of your screen to submit your relection.

HOW TO SUBMIT YOUR BADGE FOR REVIEW

1. Follow step 7 from How to upload evidence to get to this screen and select “Submit for Review”

2. You will see an alert letting you know your Microcredential is under review. You can also see a timeline of your submissions and revision requests on the right-hand side of your screen.
